How to Apply: Complete Application Checklist

Most free tattoo removal programs have similar requirements. Use this checklist to make sure you have everything ready before you apply.

Identification & Background

▢ Photo ID (state ID, driver's license, or inmate facesheet if recently released)

▢ Proof of reentry status (release date documentation or parole/probation papers)

▢ Proof of residency in program's service area (utility bill, halfway house letter, etc.)

Tattoo Details

▢ Clear, well-lit photos of each tattoo you want removed

▢ Location of each tattoo (face, neck, hands, arms, etc.)

▢ Approximate size of each tattoo (e.g., "size of a business card")

▢ Colors in each tattoo (black, color, or both)

▢ Age of each tattoo (most programs require tattoos to be 6+ months old)

Support & Advocacy

▢ Signed recommendation letter from your advocate (see template below)

▢ Advocate's full name, title, agency, phone number, and email

▢ Advocate's letter must be on official agency letterhead

Personal Commitment

▢ Written personal statement explaining why you want removal and your career goals

▢ Community service hours log (if required by the program—typically 10-20 hours)

▢ Medical history form or readiness to discuss health conditions

💡 The 6-Month Rule

Many programs require that you've been out of custody for at least 6 months before you're eligible. Use this time to gather your documents, complete any required community service, and get your recommendation letter ready.

Recommendation Letter Template

Most free removal programs—including Removery's INK-nitiative and Homeboy Industries—require a recommendation letter from an official advocate. This can be your parole officer, case manager, reentry counselor, social worker, or faith leader.


Instructions: Copy this template and give it to your advocate. Ask them to print it on their official agency letterhead, fill in the bracketed sections, sign it, and return it to you.

Tips for Writing Your Personal Statement

Most applications ask you to explain—in your own words—why you want tattoo removal and how it will help your future. Here's how to write a strong statement:

Be Honest and Direct

You don't need to over-explain or apologize. Simply acknowledge that the tattoo represents a past chapter and that you're focused on moving forward.

Connect Removal to Employment

Programs prioritize applicants who can show that removal will directly help them get or keep a job. Mention specific industries you're targeting or jobs you've been turned down for.

Show Progress

Mention what you've accomplished since release: programs completed, certifications earned, employment history, family responsibilities, community involvement.

Keep It Short

One page or less. Programs review many applications—make yours easy to read.

Example Personal Statement Opening:

"I got this tattoo on my neck when I was 19 and running with the wrong crowd. It's been seven years, and I've completely changed my life. I completed my GED, earned my forklift certification, and have been clean for three years. But every time I apply for warehouse jobs, I see hiring managers look at my neck and lose interest. I'm asking for help removing this tattoo so my past stops blocking my future."
